Moritz Osteopathy ("I", "me", "the practice") is committed to protecting your privacy and handling your personal and health information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles, and the NSW Health Records and Information Privacy Act 2002.
The information I collect
To provide osteopathic care I collect personal and health information, which may include: your name, date of birth and contact details; Medicare and private health-fund details; your medical history, presenting complaint, medications and relevant lifestyle information; my clinical findings, the treatment provided and progress notes; and correspondence with other practitioners involved in your care.
How I collect it
I collect information directly from you — through intake forms, our conversations and physical examination — and, with your consent, from third parties such as your GP or other treating practitioners. Where it is unreasonable or impracticable to collect from you directly, I may collect it from a responsible person, such as a parent or carer.
Why I collect it
I use your information to assess and provide your treatment; to communicate with you about your care and appointments; to process payments and, with your consent, private health-fund or Medicare claims; to correspond with other practitioners involved in your care; and to meet my legal, professional and insurance obligations.
Who I disclose it to
I do not sell your information. I may disclose it to other healthcare providers involved in your care, with your consent; to Medicare, the Department of Veterans' Affairs or your private health fund to process a claim you have asked me to make; to my professional indemnity insurer or legal advisers where necessary; and where required or authorised by law.
Storage and security
Your records are stored in a secure practice-management system (Cliniko) protected by access controls and encryption; any paper records are kept securely. I retain health records for as long as the law requires — for adults, at least seven years from your last visit; for patients who were under 18, until they turn 25.
Access and correction
You may ask to access the personal information I hold about you, and to have it corrected if it is inaccurate. Please contact me using the details below. I will respond within a reasonable time. In the limited circumstances permitted by law where I cannot give access, I will explain why.
This website
This site is hosted by a third-party provider and embeds a Google Map to help you find the practice; Google may collect information in accordance with its own privacy policy. The site does not require you to create an account. If online booking or website analytics are added later, this section will be updated to describe them.

If you have a concern about how I have handled your information, please contact me first and I will do my best to resolve it. You may also contact the Office of the Australian Information Commissioner (oaic.gov.au) or the NSW Information and Privacy Commission (ipc.nsw.gov.au).
